ST. MATTHEW’S CONSERVATORY OF CLASSICAL DANCE

REGISTRATION & LIABILITY WAIVER

Release of Liability and Informed Consent

 

1. Acknowledgment of Risk: I understand that participation in dance training, including but not limited to ballet, jazz, modern, and conditioning exercises, involves inherent risks, including but not limited to falls, muscle strains, sprains, and other injuries. I voluntarily assume all such risks associated with me/my child's participation.

2. Waiver and Release: In consideration of me/my child’s participation in dance training provided by St. Matthew’s Conservatory of Classical Dance, I hereby release, waive, and forever discharge St. Matthew’s Conservatory of Classical Dance, its owners, officers, employees, instructors, agents, and affiliates (collectively referred to as "Conservatory Representatives") from any and all claims, demands, causes of action, or liability arising out of or connected with participation in any activities provided by the Conservatory. This includes, but is not limited to, injury, illness, or damages sustained while on the premises or during on-site events.

3. Medical Authorization: I authorize the Conservatory Representatives to seek medical treatment in the event of an emergency. I understand that I am responsible for all medical expenses incurred.

4. Physical Corrections – Opt-In/Opt-Out:

Dance training often requires physical correction to ensure proper alignment and technique.

These corrections may involve hands-on guidance on feet, ankles, scapular wings, hands, wrists,ribs,waist,glutes, and inner thighs. Please select your preference below:

 

☐ I OPT IN and consent to hands-on physical corrections.

☐ I OPT OUT and do not consent to hands-on physical corrections.

 

I understand that opting out of physical corrections may limit my/my child’s ability to receive full technical guidance but does not affect overall participation.

 

5. Photography & Video Release (Optional):

       I grant permission for St. Matthew’s Conservatory of Classical Dance to use photos or videos of

       me/my child for promotional purposes, including but not limited to educational materials, website,

       social media accounts, banners, and mailers.

 6. Faith-Based Inspiration Clause: I acknowledge that St. Matthew’s Conservatory of Classical Dance is a  business inspired by faith inChrist. Biblical references of historic figures may be utilized in positive   affirmations for artistic growth, and I understand that the Conservatory will not be scrutinized for such utilization. All are welcome, and being a believer in Christ is not required to audition for the Conservatory program or to book private training sessions. Everyone will be treated equally, fairly, and with respect.

 

7. Participant Agreement:

I certify that I/my child is physically fit and has no conditions preventing safe participation in

  dance/Pilates based activities. I have been advised to consult a physician if I have any concerns about

  my/my child’s ability to participate. I understand that I may withdraw from participation at any time.

I agree to all uniform standards and codes of conduct as outlined in the Conservatory Handbook and or

published on the conservatory’s website.
